What Are the Main OSHA Ladder Safety Requirements?

1. Choose the Correct Ladder
Cover the correct ladder type, suitable working height, appropriate material and manufacturer's duty rating. Worker, tools and materials should be included in the total load calculation. Never exceed the manufacturer's rated capacity.
2. Inspect the Ladder Before Use
Check for broken or missing rungs, split rails, bent components, corrosion, damaged locks or spreaders, oil, mud or slippery contamination, missing labels, and damaged feet or ropes. General-industry rules require inspection before initial use during each work shift, while the construction standard requires inspection by a competent person periodically and following events that could affect safe use.
3. Remove Defective Ladders From Service
Use "Dangerous: Do Not Use" or similar identification. Damaged ladders should not remain available while someone decides whether to repair them.
4. Use Stable, Level Footing
Do not place ladders on boxes, barrels or unstable objects. Secure or stabilise ladders where level placement is impossible. Keep the area around the base and top clear.
5. Follow the 4-to-1 Ladder Rule
For a non-self-supporting ladder, position its base approximately 1 foot away for every 4 feet of working ladder length. This produces an angle close to 75 degrees.
6. Extend the Ladder Above the Landing
Portable ladders used for access must generally extend at least 3 feet or 0.9 metres above the upper landing, or alternative securing and grasping arrangements must be provided.
7. Secure Ladders Near Traffic and Doorways
Secure the ladder or install a barricade where it could be displaced by doors, vehicles, pedestrians, machinery or site operations.
8. Control Electrical Hazards
Use ladders with nonconductive side rails where workers or ladders could contact exposed energised electrical equipment. Avoid implying that every fibreglass ladder is automatically safe around electricity.

Safe Ladder Climbing and Three-Point Contact
Face the ladder when climbing. Maintain secure contact. Use at least one hand to grasp the ladder. Keep the body between the side rails. Descend and reposition rather than overreaching. Use a tool belt, hoist line or other controlled method for tools. Do not carry objects that could cause loss of balance. Never move or extend a ladder while it is occupied. OSHA explains that the one-hand grasp requirement supports the principle commonly described as three points of contact or control.
When Is Fall Protection Required on a Fixed Ladder?
The 24-Foot Rule
For relevant fixed ladders extending more than 24 feet or 7.3 metres above a lower level, OSHA's general-industry rule establishes fall-protection requirements. Differentiate between ladders installed before November 19, 2018 and ladders installed on or after November 19, 2018, replaced ladder sections, cages and wells, personal fall arrest systems and ladder safety systems. The current published rule says new fixed ladders installed on or after November 19, 2018 must use a personal fall arrest system or ladder safety system.
Important 2026 Regulatory Update
2026 update: On April 6, 2026, OSHA proposed removing the November 18, 2036 deadline for converting affected existing fixed ladders to personal fall arrest or ladder safety systems. This was a proposed rule when reviewed in July 2026. It should not be described as a final rule unless OSHA officially finalises it. OSHA stated that the requirement affecting newly installed fixed ladders would remain unchanged under the proposal.

Workers should complete a pre-use visual check. Construction ladders require periodic inspection by a competent person and inspection after an event that may affect safe use. Organisations should document who is authorised to inspect, tag and remove ladders.
