The Lock Is Applied—but Is Everyone Protected?
Group lockout tagout becomes more complex when contractors, multiple trades, and changing shifts work under the same energy isolation. Consider a shutdown-maintenance scenario involving facility electricians, mechanical technicians, an external contractor, work extending into the next shift, and several electrical, hydraulic, pneumatic, and mechanical energy sources.
The isolation may be physically secure while the people-management system is failing. Main risks include: a contractor was not added to the group system, an incoming worker begins before applying a personal lock, an outgoing employee removes a lock too early, the coordinator cannot confirm who remains exposed, and the permit and lockbox records do not match.
What Is Group Lockout Tagout?
Group lockout tagout is a hazardous-energy-control method used when several authorised employees work under a shared isolation. Each worker maintains personal control by attaching an individual lock or tag to a group lockbox, group device, or comparable mechanism.
Clarify the difference between equipment or system isolation locks, operations or control locks, personal worker locks, information tags, and group lockboxes. A shared lock by itself is not group LOTO.
Under OSHA, the group method must provide protection equivalent to personal lockout, and each authorised employee must attach a personal device when beginning work and remove it when stopping work.
When Is a Group LOTO Procedure Needed?
Examples include several technicians servicing the same machine, multiple trades working on interconnected equipment, contractors and host-employer employees sharing an isolation, a plant shutdown covering many energy-isolating devices, work extending over more than one shift, and work involving multiple machines or energy sources.
Individual versus Group LOTO Comparison:
- Individual LOTO: One employee controls the isolation directly.
- Group LOTO: Multiple employees receive personal protection through a coordinated group system.
What Does OSHA Require for Group Lockout Tagout?
Equivalent Personal Protection The procedure cannot provide less protection merely because the work involves a large crew.
Primary Responsibility A designated authorised employee receives primary responsibility for a defined group.
Exposure-Status Accountability The responsible person must be able to determine which group members are still exposed.
Overall Coordination Where several crews, crafts, departments, or employers participate, one authorised employee must coordinate the affected workforces and continuity of protection.
Personal Participation Every authorised worker applies a personal lock or tag to the group device or lockbox before beginning work. These obligations are set out in OSHA 1910.147(f)(3), which specifies that group lockout and tagout must provide each worker with a level of protection equivalent to that provided by a personal lockout or tagout device.
Who Is Responsible During Group LOTO?
Site or Facility Manager Provides the site energy-control programme, equipment information, resources, and enforcement.
Group LOTO Coordinator Controls the overall isolation, verifies procedure completion, tracks participant status, and prevents premature restoration.
Contractor Supervisor Confirms worker competence, coordinates with the host employer, and ensures contractor compliance with the agreed procedure.
Authorised Employee Reviews the isolation, applies a personal lock, verifies protection, follows the procedure, and removes the lock only when no longer exposed.
Affected Employee Understands that equipment is unavailable and does not operate, reset, bypass, or interfere with the isolation.
How Does a Group Lockbox Work?
The sequence involves isolation locks applied, isolation keys secured in lockbox, each authorised worker applies a personal lock, and the box cannot be opened until personal locks are removed.
The lockbox controls access to the isolation keys. Each employee's personal lock controls that employee's protection. A roster, signature, permit, supervisor's lock, or coloured tag does not automatically replace the personal protection required by the applicable procedure.
Group Lockout Tagout Procedure:
Step 1: Define the Work Scope and Responsible Coordinator Record equipment, crews, contractors, permit numbers, boundaries, and expected duration.
Step 2: Identify All Hazardous Energy Sources Include electrical, mechanical, hydraulic, pneumatic, thermal, chemical, pressure, gravity, and stored energy.
Step 3: Notify Affected Personnel and Shut Down Equipment Use an orderly shutdown that does not introduce a new hazard.
Step 4: Operate Every Required Energy-Isolating Device Do not rely only on emergency stops, push buttons, software controls, or selector switches.
Step 5: Apply System Isolation Locks and Control Stored Energy Bleed pressure, discharge capacitors, block elevated parts, restrain movement, and address possible reaccumulation.
Step 6: Place Isolation Keys in the Group Lockbox The coordinator records the isolation points and secures the keys before workers apply personal locks.
OSHA requires isolation of all relevant energy sources, control of stored energy, and verification before work begins. Professionals responsible for complex isolations can develop practical lockout/tagout skills through comprehensive training programs.
Step 7: Each Worker Applies a Personal Lock No authorised worker begins servicing until personally included in the protection system. Record worker name or identification, employer or contractor, work area, time of application, and lock identification where required.
Step 8: Verify Energy Isolation The authorised person should attempt the normal start control where appropriate, test electrical absence using suitable procedures and competent personnel, check pressure, movement, temperature, or process indicators, confirm stored energy is controlled, and return controls to a safe position. A lock indicates control has been applied; verification confirms whether the equipment is actually safe.
Step 9: Maintain Worker Accountability Update the roster or status board whenever someone joins, leaves, pauses work, changes tasks, or transfers to another shift.
Step 10: Release the Group Lockout Safely Before restoration: inspect equipment and work areas, remove tools and temporary devices, confirm components are operationally intact, verify that all workers are clear, reconcile every personal lock and participant, notify affected employees, remove isolation locks under the authorised procedure, and restore energy in a controlled sequence.
OSHA generally requires the person who applied a device to remove it. An employer-directed exception requires a documented procedure, confirmation that the employee is absent, reasonable contact efforts, and confirmation that the employee knows about the removal before returning to work.
How Should Contractor LOTO Be Coordinated?
OSHA requires the on-site and outside employers to inform each other of their respective lockout or tagout procedures. The on-site employer must also ensure its employees understand and comply with the relevant restrictions.
Pre-Job Contractor Coordination Checklist
Exchange written LOTO procedures, define which procedure governs the work, identify equipment and energy boundaries, assign the overall coordinator, verify authorised-worker training and competence, agree on personal-lock identification, review permit-to-work interfaces, establish communication methods and language support, define shift-transfer arrangements, define absent-worker and emergency lock-removal rules, and confirm who can suspend, revalidate, or close the permit.
Contract language can allocate activities, but it should not create gaps where each employer assumes the other has controlled the hazard.
How Is LOTO Safely Transferred Between Shifts?
Present an eight-step handover: stop work at a controlled point, review the work completed and remaining, confirm the equipment remains isolated, reconcile locks, keys, permits, and participants, brief the incoming authorised employees, incoming employees apply personal locks, outgoing employees remove personal locks only after protection has transferred, and record the transfer and confirm the new responsible coordinator.
The incoming worker's protection must be established before the outgoing worker's protection is withdrawn. OSHA requires procedures that maintain continuity through an orderly transfer between off-going and oncoming employees.
Can LOTO Remain in Place Overnight?
Yes, equipment may remain isolated during extended work when the employer's procedure continues to provide effective control and accountability. The key question is not simply whether a lock remains attached, but whether worker exposure is accurately tracked, personal locks are managed under the written procedure, the permit remains valid, shift responsibility has been transferred, the isolation has not been altered, and incoming workers verify their protection.
